Q. How is Stockholm3 different from a standard PSA test?
A PSA test measures one protein. Stockholm3 combines several protein markers, over 100 genetic markers and clinical information in a validated risk algorithm, producing an individual risk score for clinically significant prostate cancer. In published research it detected significant cancers in some men with "normal" PSA levels and helped reduce unnecessary follow-up in others.
Q. Who is this test suitable for?
Men aged 45–74 who want a more detailed risk assessment — particularly those with a family history of prostate cancer, previous borderline PSA results, or those weighing up whether further urological investigation is right for them. It is not suitable for men already diagnosed with prostate cancer.
Q. What does my Stockholm3 score mean?
The report classifies your risk of clinically significant prostate cancer and gives a clear recommendation — for example, routine follow-up or referral for further assessment such as MRI. Our nurse will explain the report, and any recommendation for further investigation should be taken to your GP or a urologist.
Q. Why does it take two weeks?
The combined protein and genetic analysis is carried out in a specialist accredited laboratory using the validated Stockholm3 algorithm. The additional analysis time reflects the depth of the assessment.
Q. Does a low-risk score guarantee I do not have cancer?
No screening test can offer a guarantee. A low-risk score indicates a low probability of clinically significant prostate cancer at the time of testing. You should continue to be aware of urinary symptoms and see your GP if anything changes.
Clinical Protocol
Avoid ejaculation and vigorous exercise for 48 hours before the test. Tell us about any prostate medication (such as finasteride or dutasteride), recent urinary infection or prostate procedures, as these affect interpretation. Results take approximately two weeks as analysis is performed in a specialist laboratory.

The evidence behind Stockholm3
Stockholm3 was validated in the STHLM3 study of nearly 60,000 Swedish men, published in The Lancet Oncology, and has since been evaluated in further European studies. Compared with PSA testing alone, the combined model improved detection of clinically significant cancer and reduced the number of men sent for unnecessary biopsy. As with any screening tool, it assesses probability rather than providing a diagnosis — but it represents a meaningful step beyond the single-marker PSA approach.
Who should think carefully before testing
Screening is a personal, informed choice. If you already have urinary symptoms — difficulty passing urine, blood in urine or semen, bone pain or unexplained weight loss — you should see your GP directly rather than waiting for screening results. Men on 5-alpha-reductase inhibitors should tell us, as these medications lower PSA and affect interpretation. Our nurse will discuss suitability before any sample is taken.
From result to action
Around two weeks after your visit you will receive your Stockholm3 report with a clear risk classification and recommendation. If the score suggests further assessment, your written report will set out exactly what to discuss with your GP or urologist, and we can provide copies of all results for your NHS record. Most men receive reassuring results and a sensible interval for re-testing.
Important: This is a screening (risk assessment) blood test, not a diagnostic test. No blood test can confirm or rule out cancer on its own. Results outside the reference range do not mean you have cancer, and normal results do not guarantee its absence. If you have symptoms, or your results are unexpected, we will advise you to discuss them with your GP, who can arrange further investigation where appropriate.
