Q. Why measure SHBG and Free Androgen Index, not just testosterone?
Most testosterone in blood is bound to SHBG and unavailable to tissues. A man can have a "normal" total testosterone but a high SHBG, leaving little usable hormone — or vice versa. The Free Androgen Index (testosterone ÷ SHBG) estimates the biologically active fraction, which often correlates better with symptoms.
Q. What do FSH and LH tell us?
FSH and LH are pituitary signals that instruct the testes. If testosterone is low with high LH, the testes are struggling despite strong signalling (primary hypogonadism). Low testosterone with low or normal LH points to the pituitary or hypothalamus (secondary hypogonadism). This distinction fundamentally changes what your GP or an endocrinologist would investigate next.
Q. Why is Prolactin included?
Raised prolactin suppresses testosterone production and can cause low libido and erectile difficulties. Occasionally it signals a benign pituitary growth (prolactinoma) that is very treatable once identified. It is an essential check in any thorough male hormone assessment.
Q. Could my symptoms be something other than hormones?
Absolutely — fatigue and low mood have many causes, including thyroid dysfunction, anaemia, vitamin deficiency and depression. If your hormone panel is normal, that is genuinely useful information, and our Advanced Full Body MOT can investigate the wider picture. Your report will suggest sensible next steps either way.
Q. Will you prescribe testosterone if mine is low?
No — we are a screening clinic and do not prescribe TRT. If your results suggest hypogonadism, your written report will set out exactly what to discuss with your GP, who can refer you to an NHS endocrinologist. Testosterone treatment requires proper specialist assessment, and confirmed low readings on at least two morning samples.

Book an early appointment: testosterone peaks in the morning, so samples should ideally be taken between 8am and 10am for accurate baseline readings. No fasting is strictly required, though a rested, unstressed morning sample gives the most reliable result. Tell us about any medication, anabolic steroid use or recent illness.

Online discussion of "low T" is full of exaggeration, but genuine hypogonadism is a real, treatable medical condition affecting an estimated 2–5% of men, rising with age, obesity and type 2 diabetes. The symptoms are non-specific — tiredness, low mood, reduced morning erections, decreased libido — which is exactly why measurement beats guesswork. UK guidance requires confirmed low morning testosterone plus symptoms before treatment is considered, and a full panel including LH, FSH and prolactin to establish the cause.
Reading your results — what the numbers mean together
The power of this panel is in the pattern. Normal testosterone with symptoms redirects attention to thyroid, sleep, stress or nutrition. Low testosterone with raised LH suggests testicular origin; with low LH, a central origin that may include reversible causes such as raised prolactin, obesity or medication effects. Your written nurse report walks through your individual pattern in plain English and tells you precisely what to take to your GP if follow-up is warranted.
