Q. What is the difference between Glucose and HbA1c?

Fasting glucose is a snapshot of your blood sugar right now; HbA1c reflects your average over the previous two to three months by measuring sugar attached to red blood cells. Together they detect both established diabetes and the pre-diabetic range (HbA1c 42–47 mmol/mol) — the zone where lifestyle change is most powerful.

Q. What is Microalbumin and why test urine?

Healthy kidneys keep protein in the blood. When their filters are damaged — most commonly by diabetes or high blood pressure — tiny amounts of the protein albumin begin leaking into urine long before standard kidney blood tests change. Detecting microalbuminuria early allows treatment that can slow or prevent progression to chronic kidney disease.

Q. Who should take this test?

Anyone over 40; people with a family history of type 2 diabetes; those with raised blood pressure, excess weight or a sedentary lifestyle; women who had gestational diabetes; and people of South Asian, Black African or Black Caribbean heritage, in whom risk rises earlier. It is also a sensible annual check for anyone already told they are "borderline".

Q. What happens if my results suggest pre-diabetes?

Pre-diabetes is highly reversible. Evidence from the Diabetes Prevention Programme shows structured lifestyle change can cut progression to type 2 diabetes by around half. Your report will explain your numbers and signpost you to your GP, who can refer you to the free NHS Healthier You programme.

Preparation

Clinical Protocol

A 12-hour fast is required for the fasting glucose measurement — only water during the fasting period. You will also provide a small urine sample at your appointment for the microalbumin test.

The silent years — why early detection changes everything

Type 2 diabetes typically develops over seven to ten years before diagnosis, and complications begin during that silent period: by diagnosis, up to half of patients already show early nerve, eye or kidney changes. HbA1c testing collapses that window. Caught at the pre-diabetic stage, the condition is substantially reversible through weight loss, activity and dietary change — caught late, it becomes lifelong management. Few blood tests offer a better return on twenty minutes of your time.

Your kidneys — the organ that never complains

Chronic kidney disease affects around 10% of UK adults, most of whom do not know. Kidneys compensate so effectively that blood markers such as creatinine can remain normal until half of function is lost. Microalbumin testing looks earlier in the process: protein leak is detectable years before that point, and treatments — blood pressure control, glucose control and specific kidney-protective medications your GP can prescribe — work best when started early.

Simple appointment, clear answers

One fasting blood draw, one urine sample, twenty minutes at our South Kensington clinic — seven days a week, with fasting appointments from 8am on Fridays. Your results arrive within about two working days as a written nurse report that explains your numbers plainly and tells you exactly what, if anything, to discuss with your GP.
