Understanding Diabetes Risk Assessment

Type 2 diabetes affects more than 4.4 million people in the UK, with an estimated 1.2 million more undiagnosed and some 6.3 million in the pre-diabetic range. It develops silently over seven to ten years, during which raised glucose is already damaging blood vessels, nerves, kidneys and eyes. By diagnosis, up to half of patients show early complications — which makes the case for earlier detection about as strong as preventive medicine gets.

Risk is not evenly distributed. Family history (one affected parent roughly triples risk), excess weight — particularly around the waist — sedentary lifestyle, age over 40, previous gestational diabetes, PCOS, and South Asian, Black African or Black Caribbean heritage (where risk rises earlier, from around age 25) all contribute substantially.

The hopeful part is equally evidence-based: pre-diabetes is highly reversible. Landmark prevention trials show structured lifestyle change cuts progression to diabetes by around half. A blood test is the only way to know where you stand — HbA1c reveals your three-month average glucose regardless of what you ate yesterday, and catches the pre-diabetic window (42–47 mmol/mol) where action is most powerful.

Frequently Asked Questions

What HbA1c level means pre-diabetes?

42–47 mmol/mol indicates pre-diabetes (non-diabetic hyperglycaemia); 48 and above on two occasions supports a diabetes diagnosis. Below 42 is normal — and if you are in the pre-diabetic band, the NHS Healthier You programme, free via GP referral, halves progression risk in trial data.

Why test urine as well as blood?

Microalbumin detects tiny protein leaks from kidney filters years before standard kidney blood markers change. Since diabetes is the leading cause of kidney failure in the UK, catching kidney involvement at the microalbumin stage — when treatment works best — matters enormously.

I am slim — can I still get type 2 diabetes?

Yes. Around 10–15% of people with type 2 diabetes are not overweight; genetics, ethnicity, age and visceral (internal) fat all play roles. Family history alone is a good reason to know your HbA1c.

How often should I check?

With risk factors or previous borderline results: annually. Without: every one to three years from age 40 is a reasonable rhythm, consistent with NHS Health Check intervals. Your report will recommend an interval for your situation.
